It means what it says :-) Module failed to lock user for whatever reason. As far as I can remember, _locking_ failure does not cause this, it's failed unlocking that causes locking to fail
Once unlocking fails, io sets up timer that tries to unlock user every 30secs.... until unlocking succeeds, after which locking is allowed again.
