Default Custom Commands

I'm still playing with the custom commands stuff and being able to organize things is much nicer now. The only problem I've got is the following:

Folder
1. Item
2. Folder
3. Item

Given that setup I can't actually get items 1 or 3 into Folder 2 because it thinks I want to change the order and not the hierarchy. There are times when that is exactly what I want to do, so I'm not sure how you can tell the difference. I can get around this by moving the item into another folder at a different level and then moving it where I want, but I'm not sure what would happen to someone trying to create folders and move items at the top level without any existing content because that wouldn't be an option...

Is there some dialog option to enable to fix this now that you've modified the way it works? Like hovering an item over the folder changes the meaning after a visible cue or something? Holding down the shift key, etc? Whatever the answer it's probably worth documenting.
